Good day! I submitted all the required documents for COVID-19 compensation claim a year ago on the website(s) provided, however, I did not receive a confirmation back then. Also, I received no update since. May I follow up the status of my claim? Thank you!

1. Name of the Hospital or Health Facility  you are currently employed?
2.  Did the facility submit the documentary requirements for Health Emergency Allowance (HEA). If so, which DOH office did they submit the requirements to?
