I am Wayne Chryzzel G. Luna, a fifth year B.S. Architecture student at Rizal Technological University, City of Mandaluyong and currently working on my thesis titled “EUNOIA: A SPECIALIZED HOSPITAL FOR PSYCHIATRIC TREATMENT AND RECOVERY INTEGRATION OF HEALING ARCHITECTURE, BACOOR, CAVITE” as a part of the requirements in Architectural Design 9.

I would like to ask to support me in pursuing this study by giving me a copy of the following:

1.) Endorsement Certificate: wherein the proponent’s project is willingly supported by your good office, only by hypothetical means.

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that DOH does not issue any endorsement letter/certificate to support a thesis. Based on the target locality of your thesis, we ask that you coordinate with the Bacoor LGU for this request
