Hi, i would like to request an updated of my vaccine certificate. I will be using the details to travel abroad.
 
Here are my details.
Origin:  City of Las Pinas

1st dose:  June 18, 2021
2nd dose:  July 18, 2021
1st booster: January  21, 2022

You asked for Request to update my Vaccine Certificate.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. 

kindly coordinate with the LGU where you received your vaccination and request for assistance to update the details in your vaccination certificate. The details of your vaccination certificate should be the same as the vaccination card. (date and place of vaccination)

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
