I would like to raise a concern regarding my vaccination record. I lost my vaccination card last October 13,2022. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Lapu-Lapu City, Cebu at Lapu-Lapu City Sports Complex. Unfortunately, I forgot the specific date of my doses. All I can remember is that, dose was on August 2022 and the second dose was at September 2022. As I have said above I lost my vaccination card and my bad for I wasn't able to take photo of it.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
