Hello!
I have concern regarding on my vax certificate. There's no 2nd dose indicated. It says no record found. I tried to update my vaxcert online but they are requesting a vaccination card for my 2nd dose. I already lost my vaccination card but I remember the date of my second dose. It was 1st of January Y.r. 2022 at Sm Bacoor Vaccination Center. Sadly, I can't provide a proof that I already had my second dose shot. Please help me! Badly need it, I will use it as applying for yellow card requirements.

Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.

Thank you for response! Akala ko aabutin pa ng weeks or buwan pero ang bilis niyo sumagot 3 days lang ata hinintay ko salamat. Mag raraise nalang ulit ako ng request ko kapag may tanong pa ako salamat.
