In this regard, permission is sought from your office to enable me to conduct data gathering of:

Annual number of people diagnosed with (CALABARZON and NCR)

b.	Cerebral Palsy
c.	Autism Spectrum
d.	Attention-Deficit Hyperactivity Disorder (ADHD)

f.	Parkinson's disease

Your FOI request is approved. I enclose a copy provided by DOH CALABARZON CENTER FOR HEALTH DEVELOPMENT for your reference. 

Please note that we are only able to provide some of the information you have requested for as the Non-communicable Diseases Prevention and Control Cluster (NCDPCC) and Regional Epidemiology and Surveillance Unit (RESU) have informed us that they do not have the requested data.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
