I would like to raise a concern regarding my vaccination record. I lost my vaccination card this February. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Marikina City. I didn't recall the date of my 1st and 2nd vaccination. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
