I would like to ask a digital copy for my covid-19 vaccination card because it was Lost. My full name is Chilton T. Pardillo and the brand of my vaccination is Sinovac.

You asked for Covid 19 vaccination card request.

Response to Request

Your FOI request is approved. 

Please be informed of the following options on how to avail of acceptable proof of COVID-19 vaccination in the meantime while VaxCertPh is currently experiencing Technical Issues:

1.	Check if your VaxCertPH is available at https://vaxcert.doh.gov.ph. If you can generate your VaxCertPH with no errors or missing information, you may use this certificate as proof of COVID-19 vaccination. 

2.	You may use your LGU vaccination card or International Vaccine Certificate (ICV). Kindly refer to the attached DOH Memorandum Circular No. 2022-0051 as your reference.

Your right to request a review

If you are unhappy with this response to your FOI
