While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. We can only suggest that you visit the DOH National Health Facility Registry website at http://nhfr.doh.gov.ph which is the masterlist of health facilities. 

We also suggest you visit the Health Facilities and Services Regulatory Bureau (HFSRB) website at http://hfsrb.doh.gov.ph for the licensing and accreditation requirements for health facilities and the DOH accredited health facilities
