You asked for a Request for medical assistance - Heart Angioplasty.

Response to Request

We have received your request for medical/financial assistance. We would like to inform you that the Malasakit Program Office (formerly the Public Assistance Unit of the Department of Health) no longer provides a Guarantee Letter (GL) or Referral Letter (RL). All applications for medical assistance are currently processed at hospitals under the Department of Health (DOH).

 
For your request to be met, you may go directly to the DOH or LGU hospitals that have Malasakit Centers. You can also contact the said facilities by phone or email. Just check the attached file and link below to find out the list of facilities with Malasakit Centers.
