Angoluan 10:18 AM, Jul 31, 2023

Good day,

I would like to cancel my update request of my vaxcert because I thought that my second dose was not recorded. However, it is my fault to fill in wrong date on my second dose therefore my second dose did not come out when I did my certification of vaccine. On my behalf I apologize for it. I hope you understand and have a nice day ahead.

Request number: 2006626

Sincerly yours,
