Hi Good day FOI!

I would like to ask request to DOH to update my vaccine record since my second dose does not appear or no record. I will be using the certificate to travel abroad.

Here are my details
Origin: Pangasinan

First dose: Pfizer 09/21/21 Mabini Pangasinan
Second dose: Pfizer 10/12/21 Mabini Pangasinan
Booster shot: Pfizer 06/08/22 Bautista Pangasinan

You asked for Vaccination Certificate.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
