Hi nawala ko po ang aking vaccine card nakalimutan ko rin po kailangan ako nagpavaccine po ang naaalala ko lang po ay may 2022 po. Nagcheck na rin po ako sa website kasi wala po.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.
