Hi! I would like to update my middle name on my vaccine card because the encoder lacked letter on typing my middle name.

You asked for Request of change of middle name from wrong encoding.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center. kindly coordinate with the LGU where you received your vaccination and request for assistance to correct the details in your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
