I am seafarer and traveling around the world. Have taken Pfizer vaccination at private clinic In Myanmar Lot No. FD 5996. Require to make sure of authentication and validation of vaccine to prevent my self and spreading.

You asked for Request expiry date and authentication for Pfizer Lot No FD 5996.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested.

VaxCertPH is the official digital vaccination certificate (DVC) for Filipinos and non-Filipinos vaccinated in the Philippines that may be used for international and domestic travel. It is free of charge, and complies with the WHO Digital Documentation of COVID-19 Certificates (DDCC) guidelines.

Unfortunately, your vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series, provided it has all the necessary information (personal details and vaccination details). Thank you

Your right to request a review
