Hi! My Vaccine Card got lost and I needed a copy of it because I am scheduled for an operation in Tacloban City.

BDate: June 27, 1996

I was vaccinated in Malitbog, Southern Leyte.

I don't remember the dates of the vaccination. I needed the card to proceed with the operation. Thank you so much.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. While waiting for your card, you will receive your Vaccine Certifacte via email, rest-assured that we've already coordinated with Agent 213 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2432-2433. Please wait for a call or an e-mail from them, if none, kindly call them back through the number above. 

In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
