Hi , 

Greetings! 

I hope this message finds you well, 

I am writing to request your good office for a copy of vaccine card as I have lost the original copy issued when I took my 1st and 2nd dose. 

Here is the information I recall as follows; 

Name: Alexis Rosaroso 
Age: 28 Years Old 
Birthday: November 20, 1994 

Vaccine: Moderna 
Place of Vaccination: Quezon City Hall 1st Dose /  2nd Dose Araneta 
Year: 2022  ( I cannot recall at best the specific day )  

I hope you can help me to have a new copy of my Vaccine Card.

Response to Request

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
