Hi! I would like to request a replacement for my lost Vaccination Card. I was not able to take a picture of my vaccination card and I also can't remember when I got it. The only thing I could remember is I got it in People's Park Davao City on 2021.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
