Good day Ma'am/Sir,

I would like to request for a cancellation of my vaxcert update request I created on August 15, 2023. It seems that I have put in my mother’s vaccination information instead of mine. I would like to delete the request and create a new one with my details. I hope you could attend to my concern immediately as I need this for my clinical duty in school. You asked for Cancellation of Vaxcert Update Request.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records and/or cancellation of request is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you need to cancel request, you may just diregard it or kindly coordinate with the LGU where you received your vaccination and request for assistance to let you print her vaccination certificate. 
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
