Good afternoon! I am Julian Velasco from Ateneo Law School. For my research paper, I would like to kindly inquire about the following matters regarding contact tracing:
1. What happens to the health declaration forms that customers fill up upon entering restaurants? 
2. Where do they go after we fill them up? What do the restaurants do with it?
3. How is the information that we fill up in such forms used by the government? 
4. Is the information actually useful? Does it really help in tracing covid-19?
