Reference to documentary requirements for accreditation of NVOCCs and Domestic/International Freight Forwarders (under form no. CPG-FTEB-BLAD-Form No.20/rev.04/05.19.20).

May I humbly request for a copy or data of their (1) Latest Audited Financial Statement and (2) Updated submission of Quarterly Cargo Statistics Report for the period of 2015 to 2019. 

The information to be gathered will be treated with utmost confidentiality and will be used for academic purposes only.
