Ma'am, Sir,

Good day!

I would like to humbly request financial assistance as I tested positive for COVID-19 last January 14, 2022. Then, four of my family members tested positive and was quarantined until February 15, 2022.  I did not go to work since January 9, 2021 due to myalgia and shortness of breath, and severe headache. I had to move out from my apartment as I failed to pay rent and failed to pay back my loans from workmates on the agreed time.

You asked for ECC Claim for COVID-19 Survivor (Employee).

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
