Dear Ma'am/Sir,

Good day!
I would like to request financial assistance because i got tested positive of Covid 19. 

(I got tested antigen last Aug 14,2021 and result was Positive. I got tested again for RT PCR last August 18, 2021 received the result on August 20, 2021 and the result was positive.)

Your immediate action to my request will be appreciated greatly.

Your Request

You asked for Financial Assistance for worker who got tested positive of Covid 19.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
