Good day!

Gusto ko lang pong itanong kung qualified po ako sa cash assistance for COVID-19 positive. Nagresign po ako sa last work ko po last October 2021. Nag apply po ako bilang J.O sa isang government hospital last January 14, 2022. Before po kami magstart sa work, need po yung negative swab test result. Last January 15,2021 nag positive po ako sa COVID-19 at dahil po dun di ako nakapagstart ng work dahil naka isolate po ako. Qualified pa rin po ba ako? May swab test result po ako at certification from City Health na naka isolate po ako.

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
