Ako po ay empleyado ng Asian Land Strategies Corporation na nagpositibo sa Covid-19 noong September 2, 2021 at nakaquarantine hanggang September 13, 2021. nais ko po sanang malaman kung paano ako makakakuha ng Covid-19 Financial Assistance. kami po ng aking asawa ay hindi nakakapasok sa aming trabaho dahil sa aming quarantine. Nawa po ay matugunan ninyo ang aming request. maraming salamat po. at magingat po kayo.. Godbless

Greetings!

Thank you for your request dated Sep 08, 2021 09:46:40 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Covid-19 Financial Assistance.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Your right to request a review
