Hi!

 I am hoping to get Cash assistance since I tested positive for Covid19 twice last May to June 2021. I got to use my Maxicare but I still have to pay for the Philhealth fee since I was not qualified based on their category for Covid coverage.

Since I tested twice I have to stop working for more than a month because I have to complete 2 14-day quarantines aside from the first 14-day quarantine I have to complete before doing the 1st test.

You asked for Cash Assistance for Covid19 Positive.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
