Hi I tested positive for covid-19 last April 22, 2021 in my work. From covid-19 Treatment Facility I was transferred to the hospital (April 30 to May 07) due to difficulty in breathing and I was diagnosed as covid-19 pneumonia moderate risk. My hospital bill (402,161 pesos) was paid by Philhealth and my employer thru HMO but I need to pay the excess in the coverage of my HMO with a total amount of 173,072 pesos through salary deduction for three years. I am asking for help or financial assistance to lessen the burden in paying the excess coverage of my HMO. Hope you can help me. Thank you.

You asked for Covid-19 Patient (Work Exposure).

Response to Request

    Your FOI request is approved. 

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Your right to request a review
