Approve na po yung sickness benefits ko due to covid-19, na finile ng employer ko sa sss nung Jan. 8, 2022 at may mare-received akong 6k+ , gusto ko itanong kung pwede paba kong mag apply for EC Claims? and ..

September 15, 2021 last reported for work ko kasi nag vacation leave ako ng 16-18 dahil pupunta kong province. Kaso nung september 17, 2021, pagdating ko sa probinsya, kailangan mag swab test at nagpositive ako. Applicable ba kong makakuha ng EC claims kahit nakaleave ako that time?
P.s nagwo-work po ako sa hospital dahil don po ang project namin (construction industry)

You asked for Covid-19 Cash Assistance.

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
