Dear Ma'am/Sir, this is to request for your assistance, with the Covid-19 the nation has been experiencing right now and   I myself tested positive last August 12, 2021, I also have to take into consideration the means I and my family have. All of us need to quarantine for 14 days since they are all exposed to me, my father were not able to work for 14 days, my mother is a fulltime housewife, and my sister is still studying. As such and as an eldest, I was given the responsibility to handle some finances in our household.  Hoping for your positive response regarding this matter. Thank you!

Your Request

You asked for ECC FOR COVID 19 POSITIVE/ HEALTHCARE WORKER.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Your right to request a review
