Hi Good day!

I would like to follow up my request regarding of my Cash Assistance due to Covid-19. January 20, 2021 I took my antigen test and the result is Positive, and on the same day I took also RT-PCR test and the result is also positive. I was quarantined at Quarantine Facility from January 20, 2021 to February 6, 2021.

I already submitted my requirements to your drop box last September 14, 2021. I also have the control number 2021-3F1D6A-21824. But when I check on Cash Assistance Application thru online it is already Encoded Cash Assistance Application in the system last October 9, 20
