Please assist me in processing my COVID-19 financial assistance. I had a high fever and other symptoms which started June 14, 2021. Then on June 16, 2021 I was brought to a quarantine facility by an ambulance. I experienced symptoms such as: Fever, soar throat, sneezing, headache, severe back pain and body pains, fatigue, diarrhea, vomiting, shortness of breath and sleeplessness. I tested positive last June 19, 2021 and was quarantined until July 2, 2021. I filed sick leave for 30 days for until today I am still suffering shortness of breath, severe back pain and headache.

You asked for NUP Anna Marie Boyero Request for COVID-19 financial assistance.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
