Good afternoon Sir/Madam, I am JImmy Marzan Pagbilao 52years old currently residing at Purok3 La paz Saguday Quirino Isabela and working at Quirino Municipal Hall Isabela.

I would like to requests for ECC Cash Assistance due to my covid19 illness pneumonic kidney chronic disease happened last Jan09-15 at Callang Gen Hospital and still undergo quarantine procedures at home with my wife and 2kids who are all positive too.

right now i dont have any finances to support for my family needs due to this illness.

You asked for ECC Cash Assistance Voucher for Covid Illness.

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

good afternoon sir/madam may i ask lang po pano ko po makita yung gsis ecc voucher para sa ecc cash assistance na approved na po ang aking request for ecc voucher. salamat po sa assistance..
