Good Day Sir/Mam,
I am Sheila Mae Barrientos Lagradante,I have an online application Last Nov.8,2021 with your ECC COVID CLAIMS,here is my reference number to check the status 2021-4542540-29791,and on Jan.12,2022 the status of my application said "Cash Assistance on hold,please provide scanned and clear copy of EC VOUCHER."
Now,I would like to know where i can send the required documents in order to pursue may application ?
Thank you so much and i would loved to hear your immediate response about this matter.

    -The Employees' Compensation Program (ECP) aims to provide disability or death benefits due to work-connected sickness or injury; 

    -ECP covers all civilian employees in the government with GSIS coverage including uniformed personnel and all compulsory members of the SSS;

    -SSS serves as implementing agency of ECP in the private sector whereas GSIS implements the ECP in the public sector; the Employees' Compensation Commission (ECC) serves as appeal and policy-making body of the ECP; as implementing agency in the private sector, the evaluation of EC claims resides in the SSS; 

    -EC benefits are distinct and separate from SSS and GSIS benefits; 

    -EC cash assistance serves as accessory benefits to EC disability or death benefits due to work-connected covid19;
