Nobleza 03:25 PM, Jul 12, 2021

Good day ECC,

I was tested positive for covid 19 last june 9, 2021.
I saw the requirements of GSIS to file for EC claims for non-admitted.
Question: Where do I secure the "statement of actual duties and circumstances leading to infection"?

Thank you very much
