Greetings!

Thank you for your request dated Jul 07, 2021 01:43:28 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Documents for the COVID19 Claim.

Response to Request

This refers to your query on the documents for the COVID-19 claims for government employees.
 
    Government employees need to submit the following documents to the GSIS branch nearest them. These documents are as follows:
-       Certificate of Employment stating: Inclusive dates of leave of absences or quarantine leave due to Covid-19; Date of Return to Work
-       Copy of RT-PCR test result
-       Medical Cert from attending doctor or from Municipal Health Officer from Barangay/LGU showing diagnosis, treatment and quarantine period.
 
If admitted to the hospital, the following additional documents are required.
-       statement of account from hospital (itemized list) with Philhealth ded/payment
-       Official Receipts of Payment of Hosp bill and med purchased by member with prescription
-       Hospital and/or clinical records of confinement/consultation to include discharge summary or period of med attendance of illness.
 
Please take note that you need to accomplish the EC forms:
-       Report of Injury/sickness/death (Applicable items to be completely filled out and duly signed by Head of Office)
-       Income benefit claim for payment form (All items to be completely filled out and duly signed by the applicant/employee)
-       Statement of Actual Duties and Circumstances leading to infection (For non-front liners)
