In nature of our work as a admin assistant from medical records I have shown a remarkable resilience and dedication despite a fear of becoming infected and infecting others. Last April 4, 2021 I was exposed to a person with COVID-19. 

April 5, 2021 Associate Health Clinic (AHC) of St. Luke's Medical Center Global City advice me to start quarantine, AHC considered me as a high-risk category of exposure because we were living in same house at the same time same work place. Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
