I was tested positive on june 04, 2021. went thru on my quarantine to an isolated facility in mexico pampanga. I have submitted my documents for ss sickness and ec sickness in my Company and received the approval and cash on September 8,2021. I have not yetapplied nor received my EC Cash Assistance from Dole. I was advised that I will received an approved documents from sss to file together with other documents for EC cash assistance but my company is claiming that there is no documeng that will be coming from them. May I ask where can I acquire Notice of Approval/Copy of Voucher from sss?

You asked for Covid 19 EC Cash Assistance.

Response to Request

    We hope you are doing good these days.

    Please be informed that EC Cash Assistance claims are filed at ECC via drop box or courier service.

    The  EC CA form is available at the ECC FB page.  Kindly print the same and fill out the details required there.

    The proof of approved EC sickness claim is available at SSS/member’s My.SSS/Employer’s My.SSS (portal).

    The requirements for EC CA are as follows:

    -Proof of approved EC claim from SSS
    -Med. Cert/Med. Abstract/quarantine clearance/RT-PCR test result
    -2 valid IDs

    Pls follow ECC FB page and www.ecc.gov.ph for additional ECP information.
