Good morning po... ask ko lng po sana paano mag avail po nung 10k assistance para po sa mga nagka covid n empleyado?
Nagclaim n po ako sa GSIS pero 3,600 lng po natanggap ko....
maraming salamat po and Godbless ...keep safe po tayong lahat...

Greetings!

Your Request

You asked for COVID 19 Cash Compensation Assistance.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the GSIS.

The forms are available at the GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

If you already have an approved EC sickness claim at GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Your right to request a review
