Your Request

You asked for Covid 19 Positive Benefits.

Response to Request

Your FOI request is approved. 

Q:  "what is the step-by-step filing of benefits to SSS and ECC for a Covid 19 positive and Covid 19 PUI?

What are the forms and requirements?"

A: If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Q: Is there a deadline of notification and submission of documents?

SSS Sickness notification from Employee to Employer is 5 days; Employer to SSS is also 5 days.

Deadline of submission of documents / application for EC benefits is 3 years from the date of contingency/illness.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Secretary Silvestre H. Bello III.
