Hi, I am Mary Ann A Garcia, mother of 2, my family contracted the virus back in Sept 19, we all tested positive, me, my partner and my 2 kids. I am the breadwinner of the family and I haven't been able to go back to work because of Post Covid Myocarditis. With all the maintenance and medication since we contracted the virus, it is overwhelming and insurance is already exhausted. Sick leaves have been exhausted long before we got hit with Covid. How can I avail of the ECC Assistance? Thank you

You asked for ECC Assistance for Covid Positive Employee.

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
