Hi, I would like to file a financial assistance for the period of my sickness  due to covid 19.
I was going to work Last july 23 and I don't know that I encountered person who is positive that why I tested positive for rapid Test July 29 and do a swab test. I was isolated for that time in airforce one and August 7 my swab test result was positive then transfer to the another isolation facility and 14 days quarantine there until another swab was done and resulted negative.

Response to Request

    Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
