Good day mam/sir.

Please i would like to make a follow as to the status of my Cash Assistance which i submitted at your Cebu Office last September 3, 2021.

My documents are already complete including the GSIS verification form which confirm that i am a COVID-19 positive patient.

Hope you can give me an update on my request the soonest possible time.  Thank you and God bless.
