To whom it may concern,

Do you accept SS Sickness availment as replacement to EC Sickness Application  for EC Voucher as one of the requirements since I have no  idea that those two were different from one another because the one that I filed last time with SSS is the SS Sickness not EC Sickness. Can I still claim it even it's way back 2020?

Response to Request

Have you filed EC Sickness Claim from SSS/GSIS?
    
    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim online at the ECC via:

    from Monday to Friday, 8AM to 5PM.

    The EC CA application form is available in our website / FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Hi Good Morning,

I know already the process but my concern is that I already have a SS-Sickness Benefit that time which was applied as work-related but it can't be seen as EC-Sickness instead SS-Sickness shows. Can I request/claim for EC-Sickness benefit too since I need it for Cash Assistance application or do you accept SS-Sickness benefit since I could provide supporting documents that I was positive that time and contracted the COVID at my work?
