Magandang araw ako po ay humihingi ng tulong sa tanggapan ng Employees Compensation Commission, para po sa Hospital Bill ng aking tito na si Allan Camarino. Siya po ngayon ay kasalukuyang naka confine at ginagamot sa Hospital ng Qualimed San jose del Monte Bulacan. Sa sakit na Covid-19 sya po ngayon ay Nag negative na bagamat dumaan sa pagiging Severe to Critical Condition. Alam naman po ng lahat na ang lahat ng publikong Ospital ay puno na Covid ward kaya naman sya ay na Admit at napagbigyan sa Qualimed. Ngayon po ang kanyang Bill ay 904,835 pesos. Sana po kami ay inyong matulungan.

You asked for Hospital Bill Assisstance.

Response to Request

We are sorry to hear the condition of Mr. Allan Camarino.   

The EC Program is designed to provide benefits to workers who suffered from work-connected sickness, injury or death.

If Mr. Camarino is an employee who suffered from a work-connected COVID,  he may file EC sickness claim at the nearest SSS branch.

The forms are available at the SSS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If he already has an approved EC sickness claim at SSS,  he can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
