Hi, I would like to inquire about the posted notice of ECC regarding the financial assistance to government workers who had been COVID-19 positive.

Greetings!

Thank you for your request dated Aug 07, 2021 11:24:15 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for FINANCIAL ASSISTANCE TO GOVERNMENT WORKER WHO BECAME COVID-19 POSITIVE.

Response to Request

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the GSIS branch nearest you.

The forms are available at the GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center
3. Medical Certificate or Home Quarantine Clearance issued by LGU Health Office/BHERT
4. 2 Valid IDs

If you already have an approved EC sickness claim at GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
