Good day!

My father is a senior citizen (62 years old ), SSS Member and he was diagnosed with the covid virus last March 2021. We can't process his retirement benefit due to pandemics, problems with employment history (our tentative appointment to SSS  on October 20 which is a very long time), and miscommunication between parties.  

Since we have no money for his maintenance. Kindly help to get the cash assistance .

Greetings!

Your Request

You asked for Senior Covid Patient.

Response to Request

    On the assumption that your grandfather was a covered member under the ECP and he has yet to file an EC claim in the system

    If he is an employee who suffered from a work-connected COVID,  pls file his EC sickness claim at the nearest SSS branch (for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If he already has an approved EC sickness claim at SSS/GSIS,  he can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
