Dear Sir/Ma’am,

I tested positive for covid 19 last March 4, 2021 and was admitted on the same day at a private hospital. I progressed to severe covid. Was discharged on March 14. Due to general weakness and fear, was only able to returned to work on April 28. I was not immediately aware of the ECC compensation benefits thats why i am applying only now. Hope you can help me on this. Thank you.

Greetings!

Your Request

You asked for EC compensation benefits for covid 19 positive patient.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Your right to request a review
