Good Day,

We would like to seek for cash assistance for one of my employee that results positive on his Antigen Test and categorized as COVID -19 Positive. He was a private sector worker, as part of that company to help employees it will be a great help to him since  he was not able to be compensate fully from the time of absence in his company

We already ask the DOLE regarding this matter and they prefer us to ask on your  good office about this matter.

We are hoping for your positive action regarding this matter.

Your Request

You asked for Financial Assistance for Coovid-19 Positive Employee ( Private Sector).

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs
