Just want to ask what are the procedures or processes how to file and what are the forms needed for claiming Covid-19 Financial Assistance.
And what is EC Logbook?

Your Request
You asked for Process/Procedures and Forms to be used for Covid-19 Financial Assistance.
Response to Request
Your FOI request is approved.
If you are an employee who suffered from a work-connected COVID, pls file your EC sickness claim at the SSS branch nearest you.
The forms are available at the SSS/ECC website.
EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center
3. Medical Certificate or Home Quarantine Clearance issued by LGU Health Office/BHERT
4. 2 Valid IDs
Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)
If you already have an approved EC sickness claim at SSS, you can then file an EC cash assistance claim at the ECC.
The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
As to your second question:
"What is an EC logbook and what does it contain?"
'An EC logbook is used to chronologically record the sickness, injury, or death of employees, indicating the names, dates, and places of contingency, nature of the contingency, and absences. Every employer should maintain one.
"What is the importance of EC logbook?"
"Considering that employers are no longer required to dispute the compensability of the EC claims, the obligation to maintain and to record sickness, injury or death among their employees is the most important participation of the employers to the reportorial requirements of P.D. No. 626, as amended.
"In case of non-compliance and the incident has been declared as work-connected, the erring employer shall be held liable to 50% of the lump sum equivalent of the income benefit to which the employee may be found to be entitled. (Art. 211 [c] of P.D. No. 626, as amended)"
