You asked for COVID assistance after GSIS is claimed.

Response to Request

Your FOI request is approved. 

Q: AFTER my covid benefits (check)  was released by GSIS how can I now file for financial assistance to ECC? Im in Olongapo City.  What do I need to do? Where would I file? and what documents are needed? how much is the assistance?

    A:  If you already have an approved EC sickness claim at GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

    To file for EC Cash Assistance, you may visit the ECC Regional Extension Unit nearest you is at:

        Unit 6 Lodestone Cove, MacArthur Hi-way
        Telabastagan, City of San Fernando, Pampanga
